One can provide the name of the person of your search and the date of birth. The one who request for the record will also be asked for their information and the reason for acquiring the document. The requesting party will also be required to indicate their relationship to the person whose name is on the certificate. The whole process of requesting the birth certificate can be released after 4 weeks upon request.
